In order to use our transportation service we need to know who you are, where you are and how to reach you.  We also require other information such as social security or medical card numbers in order to help qualify you for programs that may pay for your transportation.

All information collected is only used internally by ASC and is kept secure and strictly confidential.  We do not share the information you provide us with any other persons or entities.
